A self directed program that introduces the concepts of managing
type 2 diabetes
How to complete the program
You can complete the program at your own pace. The case study questions can be answered using a combination of dot points and short paragraphs. Many questions will only require dot point answers. We estimate that each module will take approximately one to two hours to complete. Once you have registered you will be able to use your log-in password which enables you to save your work and come back to it at any time. The level of support that Diabetes Outreach staff can provide will depend on your location.

As part of Diabetes Outreach’s commitment to rural and remote South Australia we can support you to complete this program. Please enter your case study answers in the boxes provided at the end of each module. We recommend you save your work as you go. When you have finished all of the case studies and completed the post survey an automatic email will be sent to Diabetes Outreach. A staff member from Diabetes Outreach will then review your answers and provide feedback. An email will be sent to you when the feedback is ready to be viewed by you online. The feedback can be viewed by going to the Print Your Case Study tab on the left hand side of the page.
If you are unsure about your answers or the module questions you can request assistance at any time by emailing diabetesoutreach@health.sa.gov.au or phone the office on 8222 6775. An educator from Diabetes Outreach will be able to talk you through the question or provide written feedback to your questions online.
On completion we will send you a certificate.
Alternatively you may have your answers assessed by a local diabetes health professional eg a credentialled diabetes educator. This is an excellent way to build partnerships at a local level and where possible this option is promoted by Diabetes Outreach. You may prefer to print a copy of the workbook - please login (or register) to activate this link. You can write your answers in the workbook and then receive feedback from your mentor. On completion please send a copy of your answers to Diabetes Outreach or a letter from your mentor stating that you have successfully completed. We can then provide a certificate of completion.

Unfortunately at this point in time staff from Diabetes Outreach are not able to provide support/feedback for health care providers who are not working in country South Australia. However, we encourage you to complete the program and on completion we can send you the answers via email. You may also have a local diabetes health professional eg a credentialled diabetes educator who can assess your answers and provide support for you to do the program.

This program is designed as an update to the basic principles of diabetes management and it has been designed for allied health professionals, nurses and Aboriginal health workers. Each module introduces the reader to a range of diabetes resources, tools and organisations.
Regardless of your professional background it is important to have a basic understanding of contemporary diabetes management and care. People with diabetes need education and support so they can be effective self managers. This program aims to increase knowledge and skills of general health professionals to support the client with type 2 diabetes.
The program can be a useful starting point for health professionals who have an interest in improving care for people with type 2 diabetes.
There are 11 modules that make up the ‘Diabetes Fundamentals’ program:

- suggested teaching tools for consumer education and peer education
- further reading
- a case study for you to complete.
