Clinical management referral points

GP for clients who have not completed ‘cycle of care’, make arrangements to conduct foot screen if not completed.
GP if client symptomatic or there are management concerns.
Psychologist or social worker for clients demonstrating significant emotional distress.
Individual diabetes educator service if not suitable for group participation or if diabetes educator feels they need further individual assessment or follow-up.
