Reducing risks module

Clinical management referral points

Refer to referring medical practitioner if participant has not completed recommended cycle of care.

If the participant has not had a foot screen or is unaware of their current foot health status, the diabetes educator should arrange to conduct a foot screen, communicate the outcomes to the participant, report outcomes to the referring medical practitioner and recommend referrals as appropriate.

If the participant has an ‘at risk’ foot and does not have a foot management plan in place or has not had appropriate diabetic foot education, arrange or recommend referral to a podiatrist and ensure follow up foot care education is provided.
