For those clients going on to a group, the following is a guide for the module.

- All participants should have been through the pre group assessment phase and identified these issues with you via the scales and/or open ended questions. You should therefore be aware of participant levels of wellbeing and diabetes related stress
- Small group discussion for 15 – 20 minutes using the exercise provided (discuss how they felt when diagnosed with diabetes and how they have dealt with diabetes since that time – have their feelings changed and if so what helped?)
- Facilitate the small groups while they are talking
- Ask each group to feedback to larger group any things that have helped to make diabetes part of their lives and record on whiteboard

- The diabetes team (including Medicare rebated services (allied health and mental health), HMR
